All our new mail files are being created with the default of Manager access level, even though the template has a default of no access. We have to edit the ACL of each new mail file to change back to no access.

Check the template for the settings that have square brackets around them, for example: [Default].
The normal settings (e.g., "Default" as opposed to "[Default]") are the access settings for the database template file itself. That is who can open the template and what access they have to that template (in a very similar manner to normal database access).
In a template, any entry with square brackets around it, [Default] or [Anonymous], are the real DEFAULT (as opposed to [Default]) options that will be applied to an NEW database created based on that template.
For example if the following are in the template:
Default Manager
[Default] No Access
Then the standard authenticated user will have MANAGER access to the database template and NO ACCESS to any NEW database created using that template's design.
Remember that while registering a new Lotus Notes user, the mail options are not displayed, so you will also need to check the "Advanced" options in the bottom lefthand corner of the "Register" dialog box, then choose the "Mail" option, and then review the "Mail file owner access" selection, as indicated below.
